Comparison between levosimendan versus beta agonists in preservation of renal function in cardiac surgery patients with low cardiac output syndrome

Background Post cardiac surgery acute kidney injury (AKI) secondary to postoperative low output syndrome (LCOS), is a serious complication. Positive inotropic agents are the main line of treatment for LCOS with different degrees improvement function & nephroprotective efficacy Levosimendan calcium sensitizer in muscles, which has comparable positive and effects that classic beta2 agonists.Objective To evaluate possible effect levosimendan as compared beta agonists patients LCOS.Patients Methods It prospective, randomized comparative study conducted at Ain Shams University Hospitals over period from December 2020 May 2021. A total 60 post were divided into two groups 30 each. Group (control) received beta-agonists (dobutamine or adrenaline) B (study) levosimendan. The incidence AKI diagnosis its progression renal failure both assessed.Results was 30% (n = 9) each group; 44% them control group 4) developed discharge ICU none discharge. At time ICU, beta-agonist 13.3%, while 0% statistically significant P value 0.038.Conclusion In comparison beta-agonists, may have better plays role decreasing LCOS. larger randomized, controlled trials recommended prove such beneficial exact mechanism.
